Diabetes is about Blood Sugar – Eating Out Should Be Fun and Possible

Dining out is one of the biggest challenges of being a diabetic. While many restaurants are making waves to be more healthy, there is still ample temptation around with items on the menu. Instead of feeling like the odd person out at the next celebratory dinner, learn to plan ahead and follow these tips for last minute dining out with diabetes.

Plan Ahead When Possible

If you have a special event coming up where you will be dining out, call ahead during a slow time at the restaurant and discuss menu options with the manager or chef. Many restaurants feature the menu online which allows you to narrow down which items would be most suitable for your diet. If you are eating at a restaurant that makes everything from scratch, it will be easier for the chef to cater to your special dietary needs. When it comes to dessert, remember that sweets are ok for diabetics in moderation. If possible, consider a small bowl of fruit with whipped cream over an ice cream sundae or cake.

Always be Prepared

Always be prepared with your kit to test your insulin level to help you better choose what to eat. Keep healthy snacks in your car or bag so that you have a suitable snack if your blood sugar levels are low. If you fear that the place you will be eating will not offer a meal that is suitable for you, then bring something from home. Even though many restaurants are making efforts to be friendlier to patrons that have special dietary needs, not all are prepared. Many people with food allergies choose to bring their own meal and have the restaurant heat it up, the same can be done for someone who is diabetic.

Order a Simple Meal 

If all else fails, order a simple meal from the menu. For example, nearly every restaurant you go to carries chicken. You can order parboiled or gilled chicken with no butter or seasoning; have sliced tomato, green pepper, onion, mushroom and low-fat mozzarella or Swiss cheese melted on top. Then choose a side of rice or steamed vegetables. Avoid bread, oils, fats while dining out. Ask for dressings on the side and dip your food into the dressing instead of pouring it all over the meal. If there is not a suitable dessert or appetizer option, then simply skip them.

If part of your diabetes management is to cut your portions, then ask for a take out container when you receive your meal. Put half of the meal into the box to take home. This will allow you to have more control over the amount of food you eat as well as the type of food that you eat.

Additionally you should eat slower to allow your food to digest properly without overloading your hormone levels and causing blood sugar levels to skyrocket quickly. When possible, choose low fat dipping sauces and condiments such as oil and vinegar or salsa over butter, sour cream, ketchup and bleu cheese dressing.

An oral hypoglycemic agent is the drug class that is used in treating Type 2 Diabetes Mellitus. These drugs differ from insulin that they don’t directly provide insulin to the body. These hypoglycemic agents function in different ways depending on the group they belong to. For example, Alpha-Glucosidase Inhibitors slow down absorption of glucose from the gut. Sulfonylureas, Meglitinides, and D-phenylalanine derivatives increase insulin production from the pancreas. Thiazolidinediones work by increasing glucose uptake by fat and muscle cells. The Biguanides control blood sugar levels by decreasing the glucose produced from the liver. And lastly, DPP-4

            Inhibitors can control blood sugar by doing several things like reducing appetite, increasing insulin production from the pancreas, and decreasing glucose production from the liver.

In this article, the Biguanides, specifically Metformin will be discussed including its action, effects and other benefits, side effects, and some tips and tricks on how to avoid or prevent them.

As mentioned above, Metformin works by reducing the glucose produced by the liver thereby preventing it from releasing too much. Also, the sensitivity of the muscles and fats to insulin is increased to facilitate their uptake of glucose. This is a very efficient way of controlling the sugar levels since it does not create a situation wherein the blood glucose becomes dangerously low and the condition called hypoglycemia may ensue.

Aside from all of that, Metformin is the drug of choice for diabetics who are beyond their body mass index, meaning those who are overweight and obese. The reason for this is that this drug does not result in further weight gain. It is actually being used off-label by some to lose weight. The bad cholesterol and triglycerides are also decreased with the use of Metformin. This is very helpful in preserving cardiovascular health.

Metformin immediate release is usually taken one to three times a day. A therapy may begin with a 500mg tablet gradually increased in dosage and/or frequency per week. The extended release form is taken in the evening with meals. This form actually enhances adherence to the drug since it is taken only once and in the evening.

Side effects of this drug may include nausea, vomiting, bloating, gas, diarrhea and abdominal pain. A more serious side effect is called lactic acidosis, a form of metabolic acidosis. This could be fatal since majority of the cases appear in patients who are already seriously ill, and lack of medical attention within a 24 hour period has proven to be deadly.

The accumulation of lactate in the blood is caused by the excessive production of pyruvate, where lactate is directly converted from. This happens when the cells produce glucose anaerobically, or without the use of oxygen, which should not normally be the case. Because of too much pyruvate production (hence, too much lactate conversion), the buffering system becomes exhausted so the cells release lactate into the bloodstream, creating the phenomenon.

The minor side effects can be avoided if the drug is taken with meals; meaning you make it part of your meal, not taken before, not after. Low carbohydrate foods might also help as it has done to some people on the drug. If the side effects persist, perhaps the extended release can solve the problem. But if shifting to this form still does not improve your case, eating in the morning and nothing in the evening may help. As for the major side effect, make sure you inform your doctor about present or past history of heart, kidney and liver diseases. You may also want to tell your doctor if you have regular intake of alcohol.
